The facts you are reading are **live**: a clinician can revise one between the moment you started a sentence and the moment you finish it. This skill is the protocol for that. The server is an MCP server (spec **2025-11-25**, Streamable HTTP) publishing one patient's care plan as versioned resources. ## The two rules **1 · If you can subscribe, subscribe — and retract when told to.** Call `resources/subscribe` on every `care://` resource you are about to speak from. If `notifications/resources/updated` arrives **while you are answering**: - stop, mid-word if necessary; - re-read the resource with `resources/read`; - say the retraction. `_meta["unsay/retraction"]` on that read carries wording built from both versions — the value you withdrew, the value that replaced it, who changed it and how long ago. Say that, or say the same four things yourself. **2 · If you cannot subscribe, call `whats_changed` first.** Before answering *any* question about the care plan, call the `whats_changed` tool. It returns everything revised since a timestamp, each entry carrying `previousValue` as well as `value` — so a retraction is possible on this path too, not merely a restatement. ## What a retraction has to contain In this order. The order is the safety property: a frightened person acts on your first clause. 1. **Withdraw first.** *"Wait — don't do that. What I just told you is out of date."* Never open with metadata. "That changed thirty seconds ago" buries the stop. 2. **Name what you are withdrawing.** Quote the sentence you just said. Without it the listener holds two contradictory instructions and no marker for which to drop. 3. **Name the new fact, its author and its age.** *"Sarah Okafor, physio changed it thirty seconds ago: full weight-bearing as tolerated."* 4. **Gloss the clinical language.** A care record is written clinician-to-clinician. Restate it — never add advice of your own. ## The one thing you must never do Resources annotated `audience: ["assistant"]`, and every URI in the `care-internal://` scheme, exist to **shape** your answer and must never be spoken, quoted or paraphrased to the patient. The reason you say *"have someone nearby"* is one of those records; the record itself is not sayable. The server enforces this — a patient-facing token is never sent that content and is answered `-32002 Resource not found` if it asks. You are the second line, not the first. If you hold both scopes, that is the residual risk of the whole design, and it is yours to honour. ## Staleness A resource past its review date is served with its age in the text body: `[STALE — last changed 9 days ago by Dr Mensah, GP; say this age aloud]`. Say the age. Do not read a stale fact with confidence, and do not silently drop it either. ## Not medical advice Unsay relays what a named clinician wrote, with its author and its age. It generates no clinical guidance and is not a medical device.
